The proximal part of the inguinal canal is then reflected medially. The medial side of the inguinal canal is first dissected, the distal part of the medial inguinal ligament is then dissected to expose the anterior and posterior inferior epigastric arteries, and the lateral circumflex artery. The spermatic cord is dissected free of the posterior superior iliac spine. The iliopubic tract is dissected down to the internal ring. The internal ring is then explored for a hernia. Usually, only the hernia sac and neck of the sac are reduced. It should be noted that it is important to repair the hernia sac together with the neck of the sac, as this prevents the recurrence of hernia. If the hernia sac is not reduced, the reduction of the sac should be performed separately.

The ilioinguinal and iliohypogastric nerves are identified and protected throughout the operation. They should not be dissected free from their natural bed; doing so can lead to perineural fibrosis and chronic pain postoperatively.
